The Care
Center is staffed 24/7 by a team of clinicians and Care Center representatives.
If the Care Center does not receive a patients vitals within a specific time frame according to their care plan they
call the patient and give them a reminder to take their vitals.
When systems
determine the patients vital signs are not in line with the expectations set forth in the care plan, the Care Center will
contact the patient to insure they are OK.
If the patient is experiencing
symptoms requiring timely intervention the Care Center will contact the provider.
Patients can be enrolled on line or by faxing a completed enrollment form to our office.
Devices are shipped by STI via FedEx Ground prepaid freight. All devices are pre-programmed and
ready for quick and easy installation.
Equipment is returned to STI
via FedEx Ground prepaid freight for cleaning, recalibration, and reprocessing.
Vitals monitoring and disease management software is integrated into most case management systems.
